Quick Start
Get NexusForm installed and your first form live on your Shopify store in just a few steps.
Step 1 — Install the app
Section titled “Step 1 — Install the app”- Go to the Shopify App Store and click Add app.
- Review the permissions and click Install app in your Shopify admin.
- You’ll land on the NexusForm dashboard — you’re ready to go.
Step 2 — Create your first form
Section titled “Step 2 — Create your first form”- From the NexusForm dashboard, click Create Form.
- Give your form a name (e.g. “Contact Us”).
- Add fields using the drag-and-drop builder:
- Text — short answers like name or subject
- Email — validates email format automatically
- Textarea — longer messages or feedback
- Select / Radio / Checkbox — predefined choices
- File Upload — accept attachments from customers
- Click Save when you’re happy with the layout.
Step 3 — Embed the form on your store
Section titled “Step 3 — Embed the form on your store”The fastest way to add a form to your store:
- In the NexusForm dashboard, open your form and click Embed.
- Choose App Block (recommended for Online Store 2.0 themes).
- Click Go to Theme Editor — this opens the Shopify theme customizer.
- Add the NexusForm block to any page section and select your form.
- Click Save in the theme editor.

Step 4 — Test your form
Section titled “Step 4 — Test your form”- Visit the page where you embedded the form.
- Fill in the fields and submit a test entry.
- Go back to the NexusForm dashboard and open Submissions — your test entry should appear.
