Quick Start

Get NexusForm installed and your first form live on your Shopify store in just a few steps.

Step 1 — Install the app

  1. Go to the Shopify App Store and click Add app.
  2. Review the permissions and click Install app in your Shopify admin.
  3. You’ll land on the NexusForm dashboard — you’re ready to go.

Step 2 — Create your first form

  1. From the NexusForm dashboard, click Create Form.
  2. Give your form a name (e.g. “Contact Us”).
  3. Add fields using the drag-and-drop builder:
    • Text — short answers like name or subject
    • Email — validates email format automatically
    • Textarea — longer messages or feedback
    • Select / Radio / Checkbox — predefined choices
    • File Upload — accept attachments from customers
  4. Click Save when you’re happy with the layout.

Step 3 — Embed the form on your store

The fastest way to add a form to your store:

  1. In the NexusForm dashboard, open your form and click Embed.
  2. Choose App Block (recommended for Online Store 2.0 themes).
  3. Click Go to Theme Editor — this opens the Shopify theme customizer.
  4. Add the NexusForm block to any page section and select your form.
  5. Click Save in the theme editor.

Step 4 — Test your form

  1. Visit the page where you embedded the form.
  2. Fill in the fields and submit a test entry.
  3. Go back to the NexusForm dashboard and open Submissions — your test entry should appear.
