NexusForm is a Shopify app that lets you build custom forms (contact forms, surveys, order requests, file uploads, and more) and embed them on your storefront — no coding required.

Which Shopify plans does NexusForm support?

NexusForm works with all Shopify plans: Basic, Shopify, Advanced, and Plus.

NexusForm is compatible with all Online Store 2.0 themes via app blocks. For vintage (non-2.0) themes, you can use the embed code or Liquid method. See Embedding Forms for details.

Can I have multiple forms on the same page?

Yes. Add multiple NexusForm app blocks to the same page template, each pointing to a different form.

Submissions are stored securely in NexusForm’s cloud infrastructure. You can view, export (CSV), and delete submissions from the NexusForm dashboard in your Shopify admin.

Yes. Go to your form’s Submissions tab and click Export. Submissions are exported as a CSV file.

What happens when I hit my submission limit?

The form shows a friendly “temporarily unavailable” message to visitors. You’ll get email warnings at 80% and 100% of your limit. Limits reset on the first of each month. See Plans & Limits for details.

  1. Check that the NexusForm app embed is enabled in your theme editor: Theme Editor → App embeds → NexusForm toggle on.
  2. Make sure you’ve added the NexusForm block to the correct page template.
  3. Clear your browser cache and try again.

NexusForm inherits your theme’s fonts and color scheme automatically. For further customization, use the block settings in the theme editor or add custom CSS in your theme.

  • Check that you’ve set up notifications for the form.
  • Add notifications@nexusform.app to your contacts or safe sender list.
  • Check your spam/junk folder.

Can I send notifications to multiple people?

Yes. When adding a notification, enter multiple email addresses separated by commas.

NexusForm is billed through Shopify’s standard app billing. Charges appear on your regular Shopify invoice.

Yes. Downgrade to the Free plan or uninstall the app at any time. There are no cancellation fees.

Your forms and submissions are retained for 30 days after uninstalling. If you reinstall within that window, your data is restored. After 30 days, data is permanently deleted.
